Schedule Your Visit

Call us at (408) 578-8010 or use our online appointment request form. We'll confirm your appointment and send any pre-visit instructions.

Complete Your Forms

Print and fill out your patient forms before your visit to save time. Forms include medical history, dental history, and insurance information.

Your First Appointment

We'll take comprehensive X-rays including a panoramic image, perform a full exam, and screen for gum disease, TMJ issues, and oral cancer.

Your Personalized Care Plan

Dr. Korb or Dr. Merat will walk you through their findings and work with you to create a treatment plan that fits your needs and budget.

Appointment Policy
